The full-time Bereavement Counselor position is a fully benefited position, which includes:
Summary:
Responsible for helping to ensure Hospice of Holland meets the regulatory requirements set in the Assists bereaved individuals in managing grief, emotional concerns and resource needs related to the bereavement process.
Responsibilities of the Bereavement Counselor include but are not limited to:
Develops, implements, evaluates and updates individualized care plans for bereaved clients based on assessments and client needs.
Provides grief counseling and education regarding grief symptomology and grief process for bereaved individuals.
Assists bereaved individuals in identifying and utilizing appropriate resources to meet environmental, financial, legal or psychological needs.
